John Watson and his team hit the road in Episode 4 of Watson to track down family members of Dr. Lubbock's cadaver from med school when she realized her cadaver exhibited the same symptoms of Cowden Syndrome – a genetic disorder that predisposes its victims to cancer – as a current Holmes Clinic patient. The investigation took the team all around Pennsylvania with an extended stop in Amish country.
John did a graduate study in the same town years prior. It was also where he proposed to Mary. While John wanted to reminisce about the early days, Mary pushed for them to finalize his divorce. He conceded by the end of the episode, but we'll see whether he can keep his word on that. Meanwhile, Dr. Lubbock needed a reality check, which came from the most unlikely place.
While the team worked on their case, Shinwell (Ritchie Coster) finally fought against Moriarty's handlers. We know the evil doctor is emotionally blackmailing Shinwell into helping keep tabs on John, but we'll see if John's right-hand man can outmaneuver the super villain. So far, Moriarty has been at least three steps ahead of both Shinwell and John and we are no closer to figuring out his ultimate motive.
Amish Country Brings Back Memories for Watson
The episode began with Watson giving the team three potential patients to take on as their next case. They settled on a overburdened lawyer with a strange dimpling on his tongue, which turned out to be an indicator of Cowden's Syndrome. The diagnosis gave Dr. Lubbock flashbacks to the cadaver she operated on in medical school. She was so haunted by the corpse's similarly dimpled tongue that Watson encouraged her to track down his identity so the team could warn his family members of the genetic condition and potentially save them from a late diagnosis.
The investigation took them to Amish country where Lubbock's anatomy patient was originally from. It also led them to his cousin, who had moved away from the community after a scandal, and the team saved her son from a rare allergic reaction that manifested in a lump on his throat. The case not only saved the boy's life, but it also reunited the woman with a community she sorely missed and needed to help her raise her son.
Returning to the Amish settlement had John in his feelings because he proposed to Mary there after a summer doing medical care in the village. It reminded him of the golden years of their relationship, but it also helped him realize that he needed to let Mary do what she felt was the best for her own happiness. So after multiple episodes of stalling their divorce, he gave in and agreed to meet with the mediator so they could get it finalized. We'll have to see how it sticks.
Shinwell Goes on Offense
While the team traveled across Pennsylvania to find the extended family of Lubbock's former cadaver, Shinwell was on a mission of his own. He's been yanked around by Moriarty and his agent (Kacey Rohl) since the premiere. They are threatening people very close to him, so he's had no choice but to do their bidding. However, he teamed up with Clyde, the lab robot, to develop a surveillance device of his own.
Moriarty's Agent gave him a new cell phone that was supposed to update Clyde with Moriarty's software and give Shinwell direct access to Moriarty's people. Luckily, Shinwell had done his own work with Clyde before he had the new phone, and he used his own device to follow the agent in hopes of getting a better idea of Moriarty's master plan.
His initial investigation was fruitless; this is the show's slowest-moving storyline. We have no idea what Moriarty is setting John up for or why Shinwell is so crucial to that plan. John either needs to recognize Moriarty's interference in his life soon, or we need some more details about where all of this is going before it gets too tedious to follow.
Ingrid Isn't Warm, But She Might Be a Great Friend
In other news, Dr. Derrian is still suspicious, but she may be a better teammate than we've given her credit for. She was the first one to call Lubbock out on faking her engagement (It's unclear whether the guys have realized Lubbock was lying at all or were just polite enough not to say anything). Ingrid cornered the poor doctor towards the end of the episode and told her to stop faking it.
It sounds harsh, but it was a necessary pep talk. Lubbock helped multiple patients get early cancer screenings and gave an entire family information they needed to live long and healthy lives, but she was still drowning in guilt. Ingrid Derrian pointed out how amazing her work was, the lives it saved, and that Lubbock needed to realize how awesome she was at being a doctor or she would forever be stuck faking an engagement to a man who didn't appreciate her. Talk about tough love, but it was what Lubbock needed to hear. And points go to Ingrid for talking to her alone instead of embarrassing her in front of the entire team. We're still curious about what Ingrid's real deal is, but she may not be as evil as initially suspected.
Watson continues Sundays at 9 p.m. ET on CBS. Episodes are available to stream the next day on Paramount+.
